Welcome Awesome Blossoms!   My name is Amanda Rennie. A little bit about myself I received my Bachelor’s of Science Degree in Nursing from Franciscan University of Steubenville, in Steubenville, OH. I have been a registered nurse for 20 + years in a variety of nursing positions. My goal as the school nurse is to keep the kids in the classroom where the learning takes place. I will strive to get to know each of the students and the needs they have to help them succeed.

WHEN ARE YOU TOO SICK FOR SCHOOL?

A child should not come to school (or will be sent home from school) if they have or have had:

A fever of 100 degrees or more in the past 24 hours (this needs to be taken before fever reducing medication has been given, ex: Tylenol, Ibuprofen or some cold medicine). 

Vomiting or diarrhea in the past 24 hours

An undiagnosed skin rash

Please report any positive strep, or influenza- (follow doctors guidelines on keeping students home after confirmation of above conditions)

Your child’s health may affect his or her learning. Therefore, health
information is important in planning for your child’s needs at school.
Health information from this form may be shared with other school staff
as needed. If there are any changes to your student’s health status during the year,
please update this form and send to the school.

Medications

Pre K-12th Grade
Before any kind of medication (prescription or over-the -counter) can be given a physician’s signed authorization and written parental authorization must be given to the school. All medication must be in original pharmacy container. Medications should be brought in and/or picked up by a parent or other adult. Medications will be stored in a locked cabinet at each school and administered according to physician’s order by licensed school nurse or trained office staff.

The licensed school nurse will notify the parent/guardian when a refill of the medication is needed at the school. Please bring the refill to school in a timely manner so the student won’t miss any doses while at school.

Exceptions:
Exceptions to this are: prescription medications that can be self-administered such as inhaler or epinephrine auto injectors. The school district must have a physician’s order and written parental authorization that states student can properly self administer the medication. The medication then has to be properly labeled for the student. The licensed school nurse will then assess the students ability to properly administer the medication.

For the safety of our students, it is recommended medication be given at home whenever possible. For example, medication prescribed three times a day can be given before school, after school, and at bedtime. However, the school also recognizes that certain situations may occur in which it is necessary for medications to be administered to a student during the regular school day. These medications will need a physician’s order and written parental authorization to be administered by licensed school nurse or trained office staff.

There is a link for the Medication order form. Please use as needed. All physician’s orders are good for the current school year, each school year a new signed physician form and written parental authorization is needed.

Immunizations

Minnesota Statues 121A.15 and Minnesota rules art. 4604.100 require that all children who are enrolled in a Minnesota school be immunized. These requirements can be waived for two reasons:

  1. Medical: This needs physician’s signature stated that your child should not get all or a particular vaccine
  2. Non-Medical: Parent/Guardian is opposed to all or a particular vaccine. Parent/Guardian signature needs to be witnessed by a notary.

The school is required to have all student’s immunization information on file BEFORE the school year begins. Your child may not begin the school without the immunization information on file.

Kindergarten entrance

  1. 5 DTap (5th shot not needed if 4th was after age 4)
  2. 4 polio (4th not needed if 3rd shot was after age 4)3.)
  3. Hepatitis B
  4. 2 MMR
  5. 2 Varicella ( shot not required if child has already had chicken pox disease)

7th grade students

  1. All Kindergarten vaccines must be completed
  2. 1 Tdap
  3. Meningococcal- One dose is required for 7th grade, a second dose at age 16 (10th/11th grade)

12th grade

  1. 2nd dose of Meningococcal is needed before starting school.
